Quick Answer
Download the official PDF: edistrict.up.gov.in/edistrictup/Services/res/selfdeclaration_hindi.pdf
Required for: Income Certificate, Caste Certificate, Domicile Certificate — one form works for all three
How to prepare it: Download PDF → Print → Fill with pen → Sign at the bottom → Scan as JPEG (under 100KB) → Upload on eSathi
No stamp paper needed. No notary needed. It has replaced the old ₹100 affidavit system.
What is the Self Declaration Form?
The Self Declaration Form (Swa Pramanit Ghoshna Patra) is a mandatory document that you must upload while applying on the eDistrict UP portal for an income, caste, or domicile certificate. The same form works for all three certificates — no need for separate forms.
In this form, you declare that the information you provided in your application is true, correct, and complete to the best of your knowledge. It is a self-certified statement — meaning you take personal responsibility for the accuracy of your information.
This form has replaced the older system of a notarized affidavit on ₹100 stamp paper. The central government itself promoted self-certification to save the estimated ₹8,000 crore that Indian citizens spent every year on affidavits. Now there is no need to visit a notary or buy stamp paper — just download, print, fill, sign, scan, and upload — entirely free.
Legal note: The form removes the cost of notarisation, not accountability. If the declared information is found to be false, it is an offence — the certificate can be cancelled and the applicant can face legal action.

The official Self Declaration Form PDF is available directly from the eDistrict UP portal:

Download PDFYou will also find the link on the edistrict.up.gov.in homepage — look for "Swapramanit Ghoshna Patra" or "Self Declaration Form" under quick links or the downloads section.
Do not take the form from unofficial websites. The system only accepts the official form from edistrict.up.gov.in. A different format or layout can cause problems in the application and may get it flagged or rejected during verification.
How to Fill the Form — Field by Field
The form is straightforward but must be filled carefully. Every field is explained below:
Field 1: Applicant's Name
Write your full name exactly as it appears on your Aadhaar card. Any mismatch (between the form, Aadhaar, and eSathi application) is a reason for rejection. If Aadhaar says "Ramesh Kumar Verma", do not write "R.K. Verma" or "Ramesh Verma".
Field 2: Father's / Husband's Name
Write your father's name (or husband's name for married women) exactly as given in the application.
Field 3: Full Address
Village or ward, block or tehsil, district, state (Uttar Pradesh), PIN code. It should match the address entered in the eSathi application form.
Field 4: Purpose of the Certificate
Write which certificate you are applying for. For example: "Income Certificate", "Caste Certificate", or "Domicile Certificate". If applying for multiple certificates together, you can list all of them on one form.
Field 5: Declaration
The form has a pre-printed statement in Hindi that the information given is true and that false information may lead to legal action. You do not need to write this — it is already printed. Just read and understand it.
Field 6: Date
Write the current date in DD/MM/YYYY format. It should be the date you submit the application, or close to it.
Field 7: Signature
Sign by hand with a pen. This is the most important part. A form without a signature is automatically rejected. The signature does not need to match any other document — it just needs to be genuine.
How to Scan and Upload It Correctly
Most rejections happen right here. Follow these steps carefully:
Step 1: Print the Form
On clean white A4 paper. Black & white print is fine.
Step 2: Fill with a Dark-Coloured Pen
Black or dark blue pen. Do not use pencil, light colours, or markers. After scanning, the text should be clearly readable.
Step 3: Sign at the Bottom
A clear signature with the same pen.
Step 4: Scan the Form
Do not take a photo. Use a scanner (any photocopy shop or CSC). Settings: Black & white mode, 200 DPI resolution. This gives a clear, readable image at a smaller file size.
Step 5: Save as JPEG
The eSathi portal requires JPEG format for this document — not PDF. If the scanner saves it as PDF, convert it to JPEG using any free online converter.
Step 6: Check the File Size
It should be under 100KB. If it is larger, compress it with a free online image compressor (search "compress JPEG to 100KB"). After compressing, open the file and check that the text is still readable — a blurry scan gets rejected.
Step 7: Upload It
On the eSathi portal, while filling the certificate application, upload it in the "Self Declaration Form" upload slot.
Don't have a scanner? Use a phone scanning app — Adobe Scan, CamScanner, or Google Drive's built-in scan feature. Place the form on a flat surface in good lighting. Hold the phone directly above it (not at an angle) — the app will auto-crop and enhance it.
Top 5 Reasons for Rejection
1No Signature on the Form
The most common, most preventable mistake. Everything filled, scanned, uploaded — but the signature was forgotten. The Lekhpal or Tehsildar rejects the application on seeing a declaration without a signature.
Fix: Before uploading, always check whether the signature is visible in the scanned image.
2Wrong File Format — PDF Uploaded Instead of JPEG
The eSathi upload slot expects a JPEG image file, not a PDF. Uploading a PDF can cause an error or processing failure.
Fix: Save/convert the scan to JPEG (.jpg) before uploading.
3File Too Large — Over 100KB
The system strictly enforces the 100KB limit. A colour scan of a full A4 page easily crosses 100KB.
Fix: Scan in black & white at 200 DPI (not colour, not 300 DPI). Compress with a free online tool if needed.
4Blurry or Unreadable Scan
The file was over-compressed, or a tilted photo was taken in poor lighting. The verifying officer cannot read the text.
Fix: After compressing, zoom in and check that every field is readable. If not, rescan at slightly better quality and then compress.
5Name Doesn't Match Aadhaar and the Application
"Rakesh" on the form, "Rakesh Kumar" on Aadhaar, "Rakesh K." on eSathi — three different versions create a mismatch flag.
Fix: Write the exact same name everywhere, exactly as printed on your Aadhaar card.

Frequently Asked Questions
Where do I download the Self Declaration Form?
From the eDistrict UP portal: edistrict.up.gov.in/edistrictup/Services/res/selfdeclaration_hindi.pdf. Do not take forms from unofficial websites.
Is a stamp paper or notarisation required?
No. The Self Declaration Form has replaced the notarized affidavit. No ₹100 stamp paper needed, no notary needed. Just print, fill, sign, scan, and upload.
Is one form used for income, caste, and domicile all three?
Yes. The same form is accepted for all three certificates. Mention in the "purpose" field which certificate you are applying for.
Can I fill it by typing on a computer?
Yes, you can type the details in a PDF editor and print it — but the signature must be done by hand, and the final upload must be a scanned JPEG under 100KB.
Is it necessary to fill it in Hindi?
The official form is in Hindi. But you can write the details in either Hindi or English — the fields accept both. What matters is that the information matches your eSathi application.
A CSC is charging ₹50 to fill this form — is that right?
Filling the form yourself is free — it is a free PDF download. Some CSC operators charge a facilitation fee for form-filling assistance. But the form itself has no cost. Fill it yourself if you can, and save the money.
My application was rejected because of this form — what went wrong?
The most common reasons: no signature, PDF uploaded instead of JPEG, file over 100KB, blurry scan, or name not matching Aadhaar. Fix the specific issue and reapply.
